The Registration Process Overview
The first step for enrolling your child in the PLP is the application. To be eligible, you must be a Mason employee, student, or essential worker, and your child must be between the ages of 6-12.
Complete the following application forms using Dynamic forms if you are Mason affiliated:
- Immunization and Physical Acknowledgement
- Medical Authorization to Treat
- Pick Up Authorization
- Participant Conduct Agreement
- Media Releases
- Self Administration of Medicine
Complete these application forms if you are a non-Mason essential worker. Note you will need to make a Dynamic Forms account:
Once your child’s application has been approved, you may register them for the PLP. You may register for dates up until May. Registrations closes 3 days before each session.
- Morning – 7:30am – 12:15pm
- Afternoon – 12:30pm – 5:30pm
- Full Day – 7:30am – 5:30pm
To register for full day, parents should select both Morning and Afternoon on a given day.
- One time application fee – $30
- Half day price $15
Upon arrival at the PLP, parents should have confirmation of their registration and payment. Parents must also present the confirmation email from their child’s Covid-19 screening form for the day. If a parent has multiple children attending, they will need to fill out a screening for all children attending.
